Athletic Forms
Students participating in athletics are required to have the following forms completed with parent signature on file in the HCA Athletic Office for each year.
Athletic Packet Forms:
- Current Athletic physical (valid for 1 year from date of physical)
- CIF Code of Ethics
- Emergency/Consent Form
- Policy Against Hazing
- Trips to Athletic Events
